Compounded GLP

Titan is pleased to announce that we are once again able to offer compounded GLP medications through Belmar Pharmacy’s mail-order service.

We had temporarily stopped offering compounded GLP medications not because the pharmacy “couldn’t make” them anymore, but because of regulatory rules. That issue has now been clarified, and we are moving forward with prescribing compounded GLP again. (For more information see below)

Important details:

- Titan is not a weight loss clinic. We will only prescribe GLP medications for medically necessary weight loss, defined as:

- BMI ≥ 30, or

- BMI ≥ 27 with at least one qualifying weight-related comorbidity (such as hypertension, dyslipidemia, sleep apnea etc.).

*These criteria apply to NEW prescriptions. If you are already on an GLP and require maintenance/continuation of therapy you don’t have to currently meet these criteria (for example, if your current BMI is 25 and you are on a GLP we will continue your therapy)

- Compounded GLP are not the same as branded products.

They are not FDA-approved products and are not evaluated by the FDA as brand-name GLP medications are.

- Why we are comfortable offering them:

While not identical to branded versions, the compounded formulations are similar enough in composition and potential effect that, based on current information, we are confident in resuming their use for appropriate patients.

- Pharmacy quality and safety:

All compounded GLP prescriptions will be filled through Belmar Pharmacy, a licensed, accredited compounding pharmacy.

This is the same pharmacy Titan already trusts and uses for hormone pellets and other compounded therapies.

- What this means for you:

- If you are an existing patient who was previously using compounded GLP, you may now be eligible to restart therapy, if you meet the medical criteria above.

- New or existing patients interested in GLP-based therapy can discuss whether a compounded option is appropriate and medically indicated.

As always, treatment decisions will be made on an individual basis after a clinical evaluation and discussion of risks, benefits, alternatives, and costs.

Why There Was an Interruption in Compounded GLP Availability?

You may have noticed that Titan temporarily stopped offering compounded GLP medications. This was not because the pharmacy “couldn’t make” them anymore, but because of regulatory rules and authorization limits.

In the United States, once a medication is FDA‑approved and widely available, compounding pharmacies are not usually allowed to make routine copies of that drug. An exception is sometimes made when there is a national shortage or other specific authorization. For a period of time, GLP medications (like semaglutide and tirzepatide) were in short supply, and compounding was allowed to help patients who could not access the brand‑name products. When that authorization and shortage status changed, compounding pharmacies were required to stop making those versions, and Titan had to pause prescribing compounded GLP.

That regulatory situation has now been clarified in a way that again allows certain compounded GLP products to be made and prescribed. Titan has decided to move forward with offering compounded GLP medications for medically appropriate patients only, using Belmar Pharmacy, a licensed and accredited compounding pharmacy we already trust for hormone pellets and other compounded therapies.

How to access GLP medications?

Call the office to schedule an appointment with your provider to determine your eligibility
Review the patient requirements for the GLP program
- Must sign consent for GLP indicating that you aknowledge that compounded medicatinos are not FDA approved
  Must have a credit card on file with authorization for billing for refills
- Must have a physical address on file (no PO Boxes)
Commit to follow up with your provider every 3 months to track progress, side effects, efficacy etc.

Contact your provider via Elation Passport message. Label the message "GLP refill"
- indicate that you are ready for a refill and confirm the dose (be specific)
Titan will charge your card and order the medication through Belmar Pharmacy.
Belmar will ship the medication to you overnight on ice.
- make sure someone will be home to refrigerate the medication within 24 hours.
- Belmar will not ship Thurs- Sun due to the cold requirements. Allow for 3-5 days processing through the pharmacy once Titan submits the order. This means PLAN AHEAD when requesting refills.
